Payment Breakdown:

• $50 Nonrefundable Deposit
  This deposit is required at the time of booking to reserve your appointment. It guarantees your spot and covers administrative costs.

• $150 Due at Time of Appointment
  This is payable directly to the provider during your consultation, either in person or via telehealth.

Need to Reschedule?

We understand things come up. You can reschedule your appointment with at least 24 hours’ notice. The $50 deposit is still nonrefundable but can be applied to your new appointment time.

What If I Don’t Qualify?

If the provider determines you do not meet the criteria for certification under Kentucky law, the $150 appointment fee will not be charged. Only the $50 deposit is retained.
